5. All UG(BAMS) Marksheets
6. UG Convocation Certificate
7. Internship Completion Certificate
8. Registration Certificate
9. Transfer certificate/Leaving Certificate
10. Migration certificate ( Only for Non-Karnataka students)
11. Caste certificate / income certificate ( SC, ST & OBC students)
12. Eligibility Certificate(Only for NON-RGUHS Student)
13. Photocopy of Aadhar card of the Candidate and Parents
14. 12 passport size photographs with white Background
